Key responsibilities:
* Coordinating patient self-management initiatives in a family centric program for pediatric and adolescent patients in Boston Children's Hospital's Division of Endocrinology's ADA recognized outpatient diabetes self-management education program. The program uses a multi-disciplinary approach in the management of children and adolescents with diabetes (Type 1, Type 2, steroid induced and cystic fibrosis related) and their families.
* Working with patients diagnosed with diabetes and their families, to facilitate improving self-management age specific goals for all patients; providing lifestyle coaching, and conducting one-on-one patient visits and group visits in a hospital clinic setting.
* Providing patient and family education; collaborating with outside community resources including but not limited to school and insurer-based case coordination services.
* Providing case management including oversight and interpretation of laboratory results for patients with diabetes; ordering of therapeutics, durable medical supplies and refills for prescription medications; as well as coordination of referrals and diagnostic studies as needed in consultation with the patient's physician.
Minimum qualifications:
Education:
* Bachelor of Science in Nursing required.
Experience:
* Minimum 1 year RN experience required.
* Minimum 2 years as a Diabetes Nurse Educator preferred, with a strong clinical background and experience in content areas such as patient education, diabetes, diabetes technologies (insulin pumps, continuous glucose monitors, smart pens), nutrition, physical fitness and other health related topics preferred.
* Flexibility in work schedule as needed. Ability to work at our Boston campus as well as our satellite locations
License/Certification:
* Current certification in diabetes education (CDCES) required.
* MA RN license
The posted pay range is Boston Children's reasonable and good-faith expectation for this pay at the time of posting.
Any base pay offer provided depends on skills, experience, education, certifications, and a variety of other job-related factors.
