Staff Registered Nurse – Primary Care (Full Time)

The registered nurse follows all Massachusetts Board of Registration in Nursing rules and regulations and is responsible for professional performance and direct patient care. The role covers assessment, diagnosis, planning, intervention, evaluation, and coordination of care for patients and families across the health‑care continuum, while adhering to agency policies, procedures, protocols, guidelines and standards of practice.

• Apply all steps of the nursing process to deliver timely, safe, efficient and patient‑centered care.
• Conduct systematic, ongoing assessment of patients and families, gathering overt and covert data and involving relevant stakeholders when appropriate.
• Prioritize assessment data, determine nursing diagnoses, and collaboratively establish realistic, measurable outcomes with patients and team members.
• Develop an individualized plan of care, prescribe interventions, and document all actions per departmental policy.
• Implement interventions, administer medications, and adjust plans based on ongoing reassessment and outcome evaluation.
• Educate patients and families about illness, procedures, treatments, and health maintenance, adjusting communication for diverse populations.
• Respond to emergency or crisis situations with effective decision‑making, timely communication, and appropriate delegation.
• Maintain a safe, clean unit environment, ensuring proper risk management, equipment safety, and infection control practices.
• Serve as a charge nurse or shift lead when requested and assist with orientation, precepting, and unit or departmental task forces.
• Coordinate internal and external referrals, discharge activities, and home agency services as needed.
• Act as a patient advocate, preserving dignity, confidentiality, and autonomy, and promote non‑discriminatory care for all patients.
• Collaborate with interdisciplinary team members and participate in quality improvement, research, and cost‑containment initiatives.
• Maintain professional competence through continuing education, certifications (BLS, ACLS when required), and pre‑admission and in‑service training.

• Associate’s or Baccalaureate degree in nursing (BSN preferred); non‑BSN graduates must enroll in a BSN program within six months of hire and complete it within 36 months.
• Current RN license in the Commonwealth of Massachusetts, in good standing.
• BLS credential required; ACLS (and other specialty certifications) may be required for specific units.
• Proficient in electronic health‑record systems and basic computer tasks.
• Strong interpersonal, organizational, and delegation skills to manage patient care and teamwork.
• Ability to work effectively with patients of diverse ages and backgrounds, providing care appropriate to the patient’s condition and cultural needs.
• Multilingual abilities beyond English are a plus.
• Physical stamina to meet core duties within the clinical setting.
• Effective verbal and written communication skills tailored to varied audiences.

Base salary range: $38.05 – $75.33 per hour, depending on education, experience and licensure.
