Job Summary

Greater Boston, Massachusetts – Nurse Practitioner House Calls Job. Clinical Role.

Responsibilities
  • Provide comprehensive home‑based primary care throughout the Greater Boston region.
  • Care for patients with chronic illnesses, mobility limitations, and complex medical conditions.
  • Conduct comprehensive in‑home health assessments, physical exams, medication reconciliation, and medical history reviews.
  • Diagnose and treat acute and chronic medical conditions.
  • Develop individualized treatment plans and prescribe medications as appropriate.
  • Coordinate care with primary care physicians, specialists, and multidisciplinary healthcare teams.
  • Educate patients and caregivers on disease management, preventive care, medication adherence, and lifestyle modifications.
  • Perform follow‑up visits and adjust treatment plans based on patient progress.
  • Assess urgent medical situations during home visits and coordinate appropriate interventions.
  • Maintain timely and accurate electronic medical records.
  • Participate in community outreach and health education initiatives as needed.
  • Travel throughout the Greater Boston area to provide patient care.
  • Work flexible schedules with occasional evenings or weekends based on patient needs.
Qualifications
  • Master's degree from an accredited Nurse Practitioner program (Family or Adult‑Gerontology).
  • Active Massachusetts Nurse Practitioner license (or eligible).
  • National certification through AANP, ANCC, or ONCC.
  • Experience with EMR/EHR systems preferred.
  • Spanish bilingual is a plus.
