Licensed Practical Nurse (LPN)- Rates are based on experience, case load, coverage area and skills
- Start of Care/Resumption of Care and Recertifications are paid at a higher rate

Job DescriptionSecure Home Health Care is continuously seeking and hiring per-diem nurses (LPN/RN) to conduct home visits on behalf of our Home Health Agency.

- Care coordination is paramount for us, and we ensure our patients get the best care and all clinical teams are on the same page no matter the case!
- Assess patients’ condition during every visit and chart your observations
- Perform evaluation tasks, including vital signs and medication review
- Administer medication as prescribed by the patient’s Physician
- Dress or redress open wounds and assess progress of healing
- Educate patients and their families on proper home health care strategies and procedures
- Coordinate with other care providers involved in the patient’s care plan
- Listen to the patient and respond to concerns or request
- State RN/LPN license (Required)
- CPR Certificate
- 1+ years’ experience in Home Care/VNA or inpatient or critical/palliative care
- Ability to travel to multiple patients per day (Required)
- Excellent communication, documentation and time-management skills
