• Administer prescribed medications and treatments per physician orders and nursing policies
• Assess residents, monitor for changes in condition, and report findings to the supervising nurse
• Document care provided in the electronic medical record with accuracy and timeliness
• Assist residents with activities of daily living (ADLs) and participate in rehabilitative activities as assigned
• Monitor vitals, perform routine tasks (e.g., wound care, glucose checks), and implement care plans
• Maintain a safe, clean, and therapeutic environment; observe infection control and safety protocols
• Collaborate with RNs, other LPNs, and interdisciplinary team members to ensure coordinated care

• Active Massachusetts LPN license in good standing
• Minimum 1 year of recent experience in a long-term care or skilled nursing setting (preferred)
• CPR/BLS certification preferred
• Ability to work 40 hours per week in a long-term care setting
• Strong communication and documentation skills; comfortable with basic computer use
• Eligibility to work in the United States
