Nurse Practitioner

Position Overview

Signify Health, part of CVS Health®, is seeking a Part Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations and other services in members’ homes, helping to simplify health care on a community-wide basis.

• Conduct In-Home evaluations lasting about 45 minutes, including a medication and medical history review, a physical evaluation, and optional diagnostic tests such as diabetic eye exams, spirometry, or bone density screening.
• Provide education and answer health questions during each visit.
• Fulfill the clinical duties specified for the role, including participating in diagnostic & preventive services products such as spirometry, DEE, bone density screening, etc.
• Travel locally or within licensed states and conduct virtual visits as needed.
• Work safely in diverse home environments that may contain pets, secondhand smoke, or allergens.

• Active, unrestricted license(s) in the coverage area(s).
• Board certification (required). Multi‑state licenses (preferred).
• Ability to cover primary locations such as Concord and the surrounding area.
• Qualified as a Nurse Practitioner or Physician Assistant with required credentials.

• Competitive pay: $41.85 – $90.13 per hour, base rate depends on experience, education, geography and other factors.
