Overview

The Director of Nursing (DON) is a clinical manager who assists the Area Director of Clinical Operations (ADCO) and is responsible for the supervision and oversight of all patient care services and clinical personnel in an assigned location/branch of the Long Term Care (LTC) Division. The DON strives to provide the highest quality of patient care and oversight while achieving excellent outcomes in a patient‑centered environment.

Responsibilities
  • Hire, evaluate, and terminate clinical personnel.
  • Monitor equipment for appropriate use and take steps to keep misuse to a minimum.
  • Investigate and resolve any accident, incident, lost articles, property loss, drug diversion, abuse/neglect, or unusual occurrence and initiate reports to the ADCO as necessary.
  • Provide day‑to‑day clinical management of the LTC branch branch, ensuring supervision, competency, evaluation, and planning for the clinical staff.
  • Ensure oversight clinically of patient and personnel assignments, patient care coordination, adherence to Patient’s Rights, referrals, continuous assessment of patient needs, and development, implementation, and updates of individualized care plans.
  • Adhere to all applicable regulations, accrediting standards, federal, state, and local laws, and Agency policies and procedures.
  • Be available at all times during operating hours and participate in all professional activities.
  • Supervise, guide, and develop patient care personnel, ensuring competency and scheduling.
  • Consult with the ADCO to determine staffing patterns that meet objectives and promote financial viability.
  • Institute disciplinary actions when necessary, document and follow up on them.
  • Communicate effectively with Human Resources.
  • Ensure timely orientation of new patient care personnel.
  • Coordinate interdisciplinary team services and document care coordination.
  • Coordinate case conferencing for high‑volume, high‑risk, and problem‑prone cases.
  • Meet corporate goals for clinical metrics.
  • Ensure accurate documentation and staff knowledge of best practices.
  • Train clinical staff on professional boundaries.
  • Coordinate and document care between staff, patients/families, and outside agencies.
  • Provide written instruction to patients/caregivers on visit schedules, medication profiles, treatments, and contact information for the Clinical Manager.
  • Organize patient care services, delineate responsibilities, and establish communication pathways.
  • Coordinate Agency services with community agencies and document the coordination.
  • Maintain confidentiality of patient information.
  • Attend continuing education programs and share information with staff.
  • Oversee the LTC branch QAPI program and report outcomes.
  • Assist the Director of Operations with EMS Plan activation and documentation.
  • Act as LTC branch infection control manager.
  • Review patient medical records for compliance with laws, accrediting standards, and Agency policies.
  • Assist the Clinical Supervisor in patient care conferences, staff meetings, and documentation.
  • Ensure supplies and equipment are available.
  • Use statistical data to determine quality and utilization of services.
  • Participate in personal professional growth and development.
  • Promote the goals of the Compliance Plan.
  • Ensure Agency personnel have current clinical information and practices.
  • Perform nursing functions during staff shortages and assist with staffing visits and on‑call duties.
  • Participate in other administrative functions as assigned.
  • Perform any other duties as assigned.

Education, Experience, Licenses, and Certifications
