About This Role

The RN Field Clinical Care Coordinator will play a vital role in an Integrated Care Model by advocating for members' needs and ensuring they receive appropriate services. This position is ideal for those who are passionate about improving health outcomes and enjoy working in the field.

• Location: Boston, MA
• Schedule: Full Time, Days
• Residency: Must reside in the Boston, MA market and surrounding areas
• Work Style: Field-based
• Pay: $29.00 - $52.00 per hour

• Engage members face-to-face and/or telephonically to complete a comprehensive needs assessment
• Assess, plan, and implement individualized care strategies
• Identify and initiate referrals for social service programs
• Manage the care plan throughout the continuum of care
• Partner and collaborate with internal care teams and community resources
• Communicate health-related information to ensure quality coordinated care

• Current and unrestricted independent licensure as a Registered Nurse in the state of MA

• 2+ years of clinical experience

• Current RN License in MA required
