RN Case Manager
RN Case Manager in the field providing services for mental health, substance abuse, PTSD, and aging related illness. Overall responsibilities include planning, providing and coordinating quality patient care. Overall goal of preventing readmission. Assess and develop a Plan of Care. Medication and case management. Teaching/Educating the patient and the family and other members of the nursing team.
Job Requirements:
- RN, BSN preferred
- Behavioral/Psych experience
- Home Health experience preferred
- Desire to work with underserved population
- Valid Driver’s License and ability to travel the service area
