• Manage capacity and access for patient care.
• Elicit patient information from referring MDs and outside facility nurses.
• Use triage tools and algorithms for timely patient transfers.
• Act as a liaison to ensure continuity of care for patients and families.
• Conduct initial nursing assessments including intake, diagnosis, and psychosocial evaluations.
• Collaborate with providers and support staff for comprehensive patient care.
• Document reasons for visits, testing ordered/completed, and referrals made.

Education
:

• Bachelor's Degree in Nursing required.

Experience
:

• 2-3 years of clinical nursing experience required.
• 1-2 years of patient flow, access nurse, or bed control experience preferred.

Licenses & Certifications
:

• Registered Nursing License in Massachusetts is required.
