Legacy Lifecare VNA/Hospice is seeking a Registered Nurse Case Manager (RN) to join our Boston team. This full-time role offers 40 hours per week, Monday–Friday, 8:00 AM–5:00 PM, with occasional on-call rotations and in-home or hospice setting visits.
You will deliver compassionate care, monitor residents, document changes, perform basic treatments (temperature and blood pressure), and support palliative/hospice needs including pain and symptom management. English fluency required.
