Compensation Hourly Rate:
- Registered Nurse (RN): $44.00 – $64.00 per hour DOE
- Licensed Practical Nurse (LPN): $33.00 – $48.00 per hour DOE

As a Registered Nurse (RN) or Licensed Practical Nurse (LPN) within a Senior Care Options (SCO) program, you will be responsible for delivering direct and indirect nursing care that supports the health, comfort, and independence of patients. This role involves conducting comprehensive assessments, developing and implementing care plans, coordinating with interdisciplinary teams, and educating patients and families. You will play a key role in providing high-quality, patient-centered care in a community health setting.

Day Shift - Hours per week: 40
Key Duties:
- Conduct comprehensive nursing assessments and document health data within the electronic medical record
- Analyze patient information and make informed clinical decisions within scope of practice
- Collaborate with providers to implement treatment plans, administer medications, and perform nursing interventions
- Educate patients and families regarding test results, treatment plans, medications, and health maintenance strategies
- Coordinate care with interdisciplinary team members and advocate for patients to ensure continuity and quality of care
Education

Graduate of an Accredited Nursing Program - Preferred (if applicable): N/A
Licenses & Certifications
- Required:
Massachusetts RN or LPN License; BLS Certification - Additional certifications (if applicable):
Bilingual skills preferred
Skills:
- Strong clinical assessment and patient care skills
- Excellent verbal and written communication abilities
- Ability to collaborate effectively within a multidisciplinary healthcare team
- Familiarity with personal computers and Windows-based systems preferred
