Position Overview

Health is seeking an SCO RN Care Manager to provide nursing care management across the continuum, including transitions of care, care coordination and navigation, complex case management, population health and wellness interventions, and disease/chronic condition management.

The RN Care Manager works with members, caregivers or authorized representatives, providers, and interdisciplinary care teams to develop and implement individualized care plans, address barriers to wellness, and support optimal health outcomes.

The position includes both remote/telephonic work and face-to-face visits with members, providers, and facilities.

Key Responsibilities
  • Conduct telephonic member outreach and face-to-face encounters using motivational interviewing skills to facilitate program enrollment.
  • Perform departmental assessments and evaluate members holistically to identify needs, health goals, and barriers to wellness.
  • Develop, implement, and revise member-specific care plans.
  • Complete documentation in applicable platforms according to departmental policies and regulatory standards.
  • Provide targeted health education and proactive condition-management strategies.
  • Communicate with providers and vendors involved in member care.
  • Coordinate safe transitions to the next level of care, including post-hospital discharge services and follow-up.
  • Implement strategies to help prevent readmissions.
  • Collaborate with interdisciplinary care teams, including Utilization Management, Medical Directors, pharmacy, community health workers, dementia care specialists, wellness teams, and Behavioral Health Care Management.
  • Present high-risk members at interdisciplinary rounds when appropriate.
  • Maintain professional growth through self-directed learning activities.
Qualifications Licensure and Certification
  • Current unrestricted Registered Nurse license in the state of residence.
  • May be required to obtain additional state licenses where Point
    32

    Health operates.
  • Must understand and follow applicable state Nurse Practice Acts.
  • National Case Management certification is desirable.
Education Required:
  • Bachelor's degree or relevant equivalent experience.
Preferred:
  • Bachelor's degree in Nursing.
Experience
  • Minimum 5 years of relevant clinical experience.
  • Home care or case management experience preferred.
  • Proficiency in a second language desirable.
  • Specialty-area experience is a plus.
Skills
  • Proficiency with Microsoft applications, technical concepts, and computer software.
  • Ability to work cooperatively across multiple organizational levels.
  • Skilled in assessment, planning, and member care management.
  • Advanced communication and interpersonal skills.

Work Environment and Travel
  • Must be able to work under normal office conditions and remotely as required.
  • Work involves extended periods of sitting and simultaneous use of a telephone/headset and computer.
  • Must be able to conduct face-to-face visits at:
    • Member homes
    • Provider practices
    • Healthcare facilities
  • Valid driver's license and vehicle in good working condition required.
  • Travel is required.
  • May occasionally be required to work additional hours beyond the standard schedule.
Compensation

Annual Salary: $87,091.58-$

Actual compensation is determined by factors including the scope and complexity of the role, skills, education, training, credentials, experience, and internal equity.

Colleagues may also be eligible for variable compensation, depending on company discretion and applicable program requirements.
