• Conduct comprehensive care management for a hospitalized patient population, focusing on assessment, planning, implementation, coordination, monitoring, and evaluation of services
• Develop and implement individualized care plans, coordinate with the interprofessional team, and manage transitions of care to support safe, efficient discharge
• Assess medical necessity and appropriateness of admission using Inter Qual criteria; collaborate with physicians and the care team to determine appropriate utilization
• Monitor length of stay, resource use, and patient outcomes; identify opportunities to optimize care delivery and reduce unnecessary variations
• Communicate clearly with patients, families, and external providers; participate in rounds, case conferences, and discharge planning meetings
• Document care management activities in the electronic health record (EPIC experience preferred); ensure accurate and timely data entry and care coordination notes
• May float to other units as needed to support unit operations and staffing demands

• Minimum 3 years of experience in care management, utilization management, or related acute inpatient case management
• Preferred 5+ years of experience with utilization management and care coordination
• EPIC EMR experience preferred; familiarity with Inter Qual utilization criteria
• Education:
  
  BSN is required
• Certifications:
  
  CCM or related care-management certification preferred
• Strong communication and collaboration skills; ability to work effectively in a fast-paced inpatient setting
• Willingness to float to other units as needed
