American Traveler is hiring an RN Care Coordinator for Case Management requiring an active MA RN license and Epic charting experience.

• Position within CCM Nursing unit
• Manages a caseload of patients across the episode of illness from pre-admission to post-discharge
• Coordinates care with an interdisciplinary team using patient assessments, care guidelines, protocols, and payer regulations
• Day shift schedule from 07:00-15:30
• Uses Epic EMR for charting

• Active MA RN license required; conditional or pending licenses not accepted
• Both NURSYS and BON verification required for nursing license
• AHA-issued certifications only
• Flu vaccine required with no religious or medical exemptions allowed
• Epic charting experience required

• Coordinates care to support desired clinical and financial outcomes for assigned patients
• Participates in ongoing evaluation of practice patterns and systems to improve quality, cost, and satisfaction outcomes
• Mobilizes resources to maximize efficiency of care delivery
• Local candidates residing within 60 miles of Boston are accepted at a reduced rate
• No validated parking available at Boston locations; public transportation or paid public parking required
