Job OverviewDelta‑T Group connects professionals with client opportunities within the special education, social service, behavioral health, and disability sectors. Our client has an immediate need for a Bachelor Level Case Manager in Boston, MA.
Responsibilities- Provide clients with clinical services, including case management, individual treatment or group work
- Complete all documentation including assessments, Relapse Prevention Plans, and Individual Treatment Plans
- Oversee the intake process
- Determine appropriate transition and discharge plans for all residents
- Develop individualized behavior management plans and support staff to understand the individual youth treatment concerns
- Facilitate case conferences and discharge planning for all residents
- Assist with family engagement, permanency planning and life‑skills development for all residents
- Provide on‑call support
- Bachelor's (Master's preferred) in social work or human services field
- At least 3 years of clinical experience
- Prior familiarity with Massachusetts child welfare systems and/or DCF
- Must have valid driver's license and vehicle
- Compensated weekly
- Increase or decrease your schedule at your discretion
- Ability to grow professionally
- Access to a broad array of client opportunities
