JPNDC's Family Childcare program (FCC) is a system of 115+ independent licensed early educators offering childcare in their homes to low‑income children with public childcare subsidies. JPNDC supports educators in providing high‑quality early education and care designed to prepare children for success in school and life. We also support families through an array of wraparound services and enrichment activities.

The RoleThe Sr. Case Manager will join the Family Child Care team, reporting directly to the Associate Vice President of Community Wellness. As a dedicated and skilled professional, the Sr. Case Manager will serve as the primary liaison for our priority populations, including DCF, young parents under 24, and families experiencing homelessness. In collaboration with various agency partners, the Sr. Case Manager will support individualized family needs, facilitate tailored Family Plans, and conduct developmental evaluations.
All FCC program staff members are considered mandated reporters and will be supported with the requisite training as needed. This role is ideal for an experienced social service professional who is passionate about making a positive impact in the community by supporting a staff and the families and children in the FCC program, particularly those with high needs facing challenging circumstances.
- Primary liaison for our priority population such as: DCF, Young Parents Under 24, and Families Experiencing Homelessness.
- Supervise and attends regular meetings with DCF, Shelter Systems or other family service agencies.
- Support the contractual needs of the children and families including referrals to community resources and multi‑agency reporting as required.
- Develop strategies for cross‑department referrals with JPNDC Economic Prosperity Services to develop Individualized Family Plans and goal setting plans tailored to each family's needs and goals.
- Oversee staff and perform developmental evaluations such as: ASQ's/ASQ-SE's for newly enrolled and families as needed.
- Facilitate and supports staff in parent workshops, education, self‑sufficiency, and other related enrichment activities for priority population.
- Provide families with high level and ongoing support in navigating referral processes such as:
Early Intervention, Public School Systems, Public Assistance Benefits, and/or Hospitals. - Provide one‑on‑one assistance and referrals to senior staff to help identify high‑needs families.
- Conduct routine home visits to educator programs and prepare related reports.

- Required:
- DEEC Director I certified, able to achieve Director II within 6 months.
