About The Program

PCAT-Y Outreach/Case Manager provides intensive, community‑based support to youth with significant emotional and behavioral health needs as part of a multidisciplinary team. This role focuses on engaging youth and families in services, supporting individualized treatment goals, and coordinating care across systems. It is a highly collaborative, field‑based position that requires flexibility, strong relationship‑building skills, and the ability to adapt to the evolving needs of youth and families.

Key Responsibilities
  • Deliver community‑based outreach and case management services in home, school, and other natural settings
  • Engage youth and caregivers to build consistent, trusting relationships
  • Support implementation of individualized treatment plans in partnership with the team
  • Assist with crisis prevention and intervention, utilizing a team‑based approach
  • Coordinate care with providers, including mental health, medical, educational, and state agencies
  • Support development of life skills, emotional regulation, and social functioning
  • Connect youth and families to community resources and supports
  • Participate in daily team meetings and ongoing treatment planning
  • Complete timely and accurate documentation in compliance with program standards
  • Provide transportation support for youth as needed
  • Schedule:

    Full‑time with flexible hours including evenings and occasional weekends
Qualifications
  • High School diploma or GED. Bachelor’s degree preferred.
  • Must be able to work with people of different backgrounds, languages and ethnicities.
  • Driver’s license required.
