The Social Services Designee plays a vital role in supporting residents and families throughout their stay. This position assists with psychosocial assessments, discharge planning, adjustment support, and ensuring residents receive compassionate, coordinated services.
You will collaborate closely with nursing, admissions, and administration to help residents feel safe, respected, and supported.
Key Responsibilities- Conduct psychosocial assessments for new admissions
- Provide support, counseling, and advocacy to residents and families
- Assist with discharge planning and community resource coordination
- Maintain documentation in compliance with state and federal regulations
- Participate in care plan meetings with an interdisciplinary team
- Address resident concerns and help promote a positive, respectful environment
- Support transitions of care and assist with family communications
- Experience in Social Services or Human Services in a healthcare or long‑term care setting (LTC experience preferred)
- Strong communication, empathy, and problem‑solving skills
- Ability to work collaboratively in a fast‑paced, resident‑centered environment
- Bachelor’s in Social Work (BSW) or related field preferred
- Licensed Clinical Social Worker (LCSW) or a Bachelor’s degree in Social Work required
- Minimum of 1 year of experience working in a Skilled Nursing Facility (SNF)
- Strong organizational and time management skills across admissions, care planning, discharges, and grievances
- Understanding of resident rights, abuse prevention, and mandatory reporting requirements
- Strong family engagement and conflict resolution skills
- Ability to complete psychosocial assessments, including MDS Section D (PHQ‑9) and Section Q (discharge planning)
- Competence in developing and updating person‑centered care plans
- Experience with behavioral interventions and trauma‑informed care approaches
