Stabilization Case Manager (NHC-1461498) Boston, Massachusetts
Salary: USD
24 - USD
46 per hour
A large provider for homelessnessis looking to add Case Managers to theirsupport services based team! If you are seeking the opportunity for internal growth in you social services career, this is for you!
Schedule: Monday - Friday - 40 hours per week, 8:30a-4:30p or 9a-5p
Job Summary:
As part of the Support Services Team, work with all aspects of the program, including outreach, community referrals, Rapid Resolution, intake and assessment, case management, rapid housing and housing stabilization. Develop relationships with landlords and property managers for permanent housing placement and retention.
Required Qualifications:
- Bachelor’s degree in Human Services or related field
- 1+ years community case management experience required
- Must understand and have ability to work with Harm Reduction, Motivational Interviewing and Trauma Informed Care best practices.
- Valid Driver's license
Pay Rate: $24.00 - $26.00/hr.
