Boston Medical Center’s Living Well at Home Program (LWAH) expands with a new Community Support Program for Homeless Individuals. As an HCS Specialist, you will provide housing‑focused case management to high‑risk patients, advocating for enrollment in housing and community supports to stabilize independence.
You’ll collaborate with a multidisciplinary team, engage families and communities, and help patients access social services, monitoring progress toward housing readiness and health goals.
