Organization OverviewThis independent Direct Primary Care (DPC) practice delivers proactive, longevity-focused primary care through a membership model. Operating outside traditional insurance constraints, the practice emphasizes relationship-based care supported by advanced diagnostics, personalized care planning, and long-term clinician‑patient partnerships.
Role OverviewWorking at the intersection of clinical care and operational coordination, the Clinical Care Coordinator manages triage, results follow‑up, clinical messaging, and front‑desk clinical workflows. When needed, this role also provides hands‑on clinical support, including injections or phlebotomy, to assist providers and maintain efficient clinic operations.
Key Responsibilities Patient Experience & Front-of-Clinic Support- Check patients in and out with clinical accuracy and a welcoming, professional demeanor
- Support patient needs during visits and follow‑up interactions
- Ensure smooth coordination between front desk, clinical staff, and providers
- Manage patient communications via patient portal, phone, and related channels
- Triage inbound messages and escape urgent or complex concerns appropriately
- Provide clinical callbacks to review test results, next steps, and care plans
- Serve as a coordination point for patient needs, referrals, and follow‑up care
- Organize incoming lab and diagnostic results for provider review
- Maintain accurate health maintenance and preventive care tracking
- Identify overdue, missing, or abnormal results and elevate as needed
- Process non‑controlled prescription refills
- Communicate with pharmacies regarding prescription updates
- Maintain accurate documentation and ensure provider awareness
- Perform standard MA or nursing visits, injections, phlebotomy, and other clinical tasks
- Assist providers and clinical staff during peak times or staffing gaps
- Partner closely with care team members to maintain continuity of care
- Patients feel informed, supported, and cared for between visits
- Providers trust clinical communication and follow‑up processes
- Clinical workflows are organized, calm, and efficient
- Preventive care and results tracking are accurate and timely
- The clinic runs smoothly with strong attention to detail

- 2+ years of experience in outpatient, ambulatory, or primary care settings
- Strong clinical judgment and triage capabilities

- Willingness to work on‑site in Longmont and Boulder

- Monday-Thursday: 8:00 AM - 5:00 PM
- Friday: 8:00 AM - 2:00 PM
