This role is ideal for a Medical Assistant or Licensed Practical Nurse who enjoys being deeply involved in patient care, loves keeping things running smoothly, and wants to work in a model that prioritizes people over paperwork.

Free from traditional insurance constraints, we deliver comprehensive, relationship-based care supported by advanced diagnostics, personalized care planning, and long-term clinician–patient partnerships.
Role OverviewOperating at the intersection of clinical care and coordination, the Clinical Care Coordinator manages triage, results follow-up, clinical messaging, and front-desk clinical workflows. When needed, this role also provides hands‑on clinical support such as injections or phlebotomy to support providers and maintain smooth clinic flow.
Responsibilities- Check patients in and out with clinical accuracy and a warm, welcoming demeanor
- Support patient questions and needs during visits and follow‑up
- Ensure smooth transitions between front desk, clinical staff, and providers
- Manage patient clinical communications via patient portal, Spruce, phone, and related channels
- Triage inbound messages and elevate urgent or complex concerns appropriately
- Provide clinical callbacks to explain test results, next steps, and care plans
- Sort and organize incoming lab and diagnostic results for daily provider review
- Maintain accurate health maintenance and preventive care tracking
- Proactively identify overdue, missing, or abnormal results and elevate as needed
- Refill non‑controlled prescriptions through the patient portal
- Communicate with pharmacies and support prescription updates
- Ensure accurate documentation and provider awareness
- Act as a point of coordination for patient needs, referrals, and follow‑up tasks
- Partner closely with providers and care team members to ensure continuity of care
- Perform standard nursing or MA visits, injections, phlebotomy, and other clinical tasks
- Assist providers and clinical staff during peak times or staffing gaps
- Patients feel informed, supported, and cared for (even between visits)
- Providers trust that clinical communication and follow‑up are handled thoughtfully
- Clinical workflows feel organized, calm, and efficient
- Results and preventive care tracking is accurate and timely
- The clinic runs smoothly because details don’t fall through the cracks
- Love working directly with patients and supporting their care journey

- Active Medical Assistant certification or Licensed Practical Nurse (LPN) license in good standing
- 2+ years of experience in outpatient, ambulatory, or primary care settings
- Strong clinical judgment and triage skills
- Excellent verbal and written communication skills
- Professional, warm, and patient‑centered approach
- Highly organized with the ability to manage multiple workflows simultaneously
- Willingness to work on‑site at our Longmont and Boulder clinics
