Full-time

If you love working closely with patients, believe healthcare should feel organized and human (not rushed and transactional) and want to be part of building a better primary care experience, this role is for you.

Cloud Medical is building primary care as it was always intended: rooted in relationships, prevention, and long-term health optimization. We’re looking for a Patient Care Manager who thrives at the intersection of patient care, communication, and clinical coordination—and who takes pride in creating calm, clarity, and trust for patients navigating their healthcare.

This role is ideal for a Medical Assistant or Licensed Practical Nurse who enjoys being deeply involved in patient care, loves keeping things running smoothly, and wants to work in a model that prioritizes people over paperwork.

About Cloud Medical

Cloud Medical is an independent Direct Primary Care (DPC) practice offering proactive, longevity-focused primary care through a membership model. Free from traditional insurance constraints, we deliver comprehensive, relationship-based care supported by advanced diagnostics, personalized care planning, and long-term clinician–patient partnerships.

Role Overview

Operating at the intersection of clinical care and coordination, the Clinical Care Coordinator manages triage, results follow-up, clinical messaging, and front-desk clinical workflows. When needed, this role also provides hands‑on clinical support such as injections or phlebotomy to support providers and maintain smooth clinic flow.

Responsibilities
  • Check patients in and out with clinical accuracy and a warm, welcoming demeanor
  • Support patient questions and needs during visits and follow‑up
  • Ensure smooth transitions between front desk, clinical staff, and providers
  • Manage patient clinical communications via patient portal, Spruce, phone, and related channels
  • Triage inbound messages and elevate urgent or complex concerns appropriately
  • Provide clinical callbacks to explain test results, next steps, and care plans
  • Sort and organize incoming lab and diagnostic results for daily provider review
  • Maintain accurate health maintenance and preventive care tracking
  • Proactively identify overdue, missing, or abnormal results and elevate as needed
  • Refill non‑controlled prescriptions through the patient portal
  • Communicate with pharmacies and support prescription updates
  • Ensure accurate documentation and provider awareness
  • Act as a point of coordination for patient needs, referrals, and follow‑up tasks
  • Partner closely with providers and care team members to ensure continuity of care
  • Perform standard nursing or MA visits, injections, phlebotomy, and other clinical tasks
  • Assist providers and clinical staff during peak times or staffing gaps
What Success Looks Like
  • Patients feel informed, supported, and cared for (even between visits)
  • Providers trust that clinical communication and follow‑up are handled thoughtfully
  • Clinical workflows feel organized, calm, and efficient
  • Results and preventive care tracking is accurate and timely
  • The clinic runs smoothly because details don’t fall through the cracks
You’ll Thrive at Cloud Medical If You
  • Love working directly with patients and supporting their care journey
  • Take pride in organization, follow‑through, and clear communication
  • Enjoy collaborating with providers and being a key part of the care team
  • Believe healthcare should be proactive, preventive, and relationship‑based
  • Want to work outside of traditional insurance‑driven care models
  • Feel ownership over patient experience—not just assigned tasks
This Role May Not Be a Fit If You
  • Enjoy highly siloed or purely task‑based work
  • Are most comfortable in insurance‑heavy, transactional care environments
  • Don’t enjoy juggling communication, coordination, and clinical details
  • Aren’t interested in contributing to a team‑oriented, evolving care model
Qualifications (Must‑Have)
  • Active Medical Assistant certification or Licensed Practical Nurse (LPN) license in good standing
  • 2+ years of experience in outpatient, ambulatory, or primary care settings
  • Strong clinical judgment and triage skills
  • Excellent verbal and written communication skills
  • Professional, warm, and patient‑centered approach
  • Highly organized with the ability to manage multiple workflows simultaneously
  • Willingness to work on‑site at our Longmont and Boulder clinics
