Oak Street Health seeks a Behavioral Health Specialist (LCSW, LPC, or LMFT) to provide integrated mental health services within a primary care setting for Medicare adults in underserved communities. Responsibilities include biopsychosocial assessments, brief evidence-based therapy, crisis intervention, treatment planning, and coordination with primary care providers and care teams. The specialist will track outcomes, document in the EHR, support care transitions, and provide psychoeducation.

- Provide biopsychosocial assessments and diagnostic evaluations for adult patients
- Deliver brief, evidence-based individual and group therapy
- Offer crisis intervention and safety planning as needed
- Develop and update person-centered treatment plans
- Collaborate with primary care providers and interdisciplinary care teams
- Coordinate referrals to community and specialty behavioral health resources
- Document all encounters accurately in the electronic health record (EHR)
- Monitor clinical outcomes and adjust interventions based on data
- Provide psychoeducation to patients, families, and staff
- Participate in team huddles, case conferences, and quality improvement efforts
- Diagnostic assessment and clinical formulation
- Evidence-based psychotherapies (e.g., CBT, MI, brief therapy)
- Crisis assessment and intervention
- Care coordination and interdisciplinary teamwork
- Behavioral health treatment planning
- Electronic health record (EHR) documentation
- Knowledge of community mental health resources
- Risk assessment and safety planning
- Outcome measurement and quality improvement
- Cultural competence working with diverse, underserved populations
- Active LCSW, LPC, or LMFT license in state of practice
- Master’s degree in Social Work, Counseling, or Marriage & Family Therapy
- Experience in integrated or primary care behavioral health preferred
- Training in evidence-based therapies (e.g., CBT, MI)
- Familiarity with Medicare population and chronic disease management
