Position: QMAP PRN as needed Day & Night Shifts Available $21.75 p/h

Position Summary

Personal Care Partner II (QMAP PRN as needed, Day & Night shifts). Hourly compensation $21.75. This role supports residents by providing daily personal care and medication assistance, ensuring safety, and communicating with nursing staff.

Essential Duties
  • Provide daily personal care including bathing, dressing, meals, and laundry with attention to details.
  • Monitor residents for hygiene and health needs, reporting urgent concerns immediately.
  • Maintain open communication with supervisors and teammates and keep them informed of resident issues.
  • Ensure resident safety, reporting abuse, neglect, or misuse of resources.
  • Document changes in residents’ health status and report to supervisor.
  • Administer prescribed medications according to state laws and facility procedure; verify correct resident, dose, timing, route, and document refusals.
  • Restock medication carts and supplies such as gloves and sanitizers.
  • Submit work orders for needed replacements or repairs.
  • Attend monthly staff meetings to discuss resident needs and departmental updates.
  • Perform other duties as assigned with flexibility and a positive attitude.
Basic Qualifications & Experience
  • Medication certification required: QMAP‑A/L, Med Tech, Med Aide, Nurse Delegation.
  • English language proficiency: read, write, speak.
  • High school diploma or equivalent (GED).
  • At least one year experience in elder care preferred.
  • Proficient computer skills preferred.
  • Strong self‑management, organization, and multi‑tasking skills.
  • Capability to handle difficult or sensitive issues while maintaining confidentiality.
Working Conditions /

Physical Requirements
  • Moderate physical activity: handle objects up to 50 lb occasionally, up to 20 lb frequently.
  • Constant standing, walking, grasping; frequent stooping and reaching; occasional sitting, crawling, kneeling, squatting.
  • Potential exposure to blood‑borne pathogens, body excretions, communicable diseases, and household chemicals.
  • Frequent interruptions and interaction with clients, family members, and visitors.
