Physical Therapist

Boulder Community Health is seeking a Physical Therapist who assumes responsibility and accountability for providing quality patient care through the provision of physical therapy services. Qualifications include a current Colorado state license for Physical Therapy, BLS certification required by the end of the conditional period, and experience evaluating neurologically involved patients and conditions. The Physical Therapist will develop, execute, assess, and modify physical therapy intervention as it applies to the neurologically involved patient population.

Responsibilities include evaluative skills to select appropriate tests and measures to determine goals and problems, course of treatment and appropriate therapeutic procedures/modalities. The Physical Therapist will select and establish a physical therapy diagnosis, formulate and maintain an individualized patient plan of care based on long and short-term goals, provide treatment using appropriate tools, demonstrate complete knowledge of patient management, evaluate patient response to treatment and revise plan of care as indicated, serve the patient population from adolescent to older adult/geriatric, modify assessment, treatment and interaction skills based on the age and developmental level of the patient, promote effective communication and teamwork with team members, the hospital and other contacts, report and document evaluations, plans, treatment techniques, assessment, and patient progress, initiate and be responsible for problem-solving and decision-making with relation to patient care, and function as an advocate for the patient.
