Office of Adult, Aging and Disability ServicesThe Colorado State Veterans Community Living Center at Rifle is located on Colorado's western slope in the scenic Colorado River valley. Rifle offers a mild climate, with plenty of clean mountain air and beautiful blue skies. Amidst this setting, the State Veterans Community Living Center at Rifle provides 89 beds of skilled nursing care, including a 18-bed Special Care Unit for residents with Alzheimer's Disease or related conditions.

You are required to work at your assigned work location during your scheduled work times.
Monday thru Friday, 8:00 am - 4:30 pm

The position exists to assist the Medical Records Technician in performing all functions of the health record program. This position schedules all health care appointments and coordinates the transportation schedule, as well as provide notification to family/resident of appointments. Follows up with physician offices to obtain reports for records. Maintains census reporting in absence of Medical Records Technician. Tracks physician visits (Regulatory and History & Physicals) needed to be performed by physicians on a monthly basis
Additionally, as a HCT I Transportation Aide you can expect to:- Transport residents to and from appointments outside the facility
- Communicate information with medical providers and appropriate facility staff regarding the current status of the person served
- Administers oxygen and monitors vital signs of client.
- Supervises clients in the medical clinic until the appointment is completed.
- Prepares residents for examination such as transferring residents out of their wheelchairs onto an exam bed and obtaining vital signs (temperature, pulse, blood pressure and respiration rate) and weighing the resident.
- Protects the rights of persons served and enhances their quality of life
- Provides direct resident care including bathing, dressing, undressing, assisting in and out of bed, performing personal hygiene, feeding programs, toileting programs, range of motion, transporting, and interacting socially to meet residents’ basic needs
- Explaining patient care before, during, and after contacts; answering call lights; and assessing basic resident care needs; updating noted resident changes; recommending routines and schedules to IDT Team. Report to the charge nurse resident’s change of condition
- Perform basic resident observations, vital signs, blood pressure, weight, intake and output, basic skin assessments
- Provide infection control, implementing/using universal precautions, disposal of waste. Clean linen room and equipment, stock supplies, complete supply forms.
