Salary Range: $58,011.00 To $92,394.00 Annually

The salary range for this position is Summit Stone’s good faith belief and reasonable estimate of the range of possible compensation for this position at the time of the posting. Responsible for providing clinical behavioral health services within a multi-site primary care setting, addressing the mental health, substance use, and behavioral change needs of children, adolescents, and adults. Collaborates closely with multidisciplinary teams across a Community Partner Location, Summit Stone Health Partners, and community agencies to support whole-person wellness through brief interventions, behavioral health screenings and assessments, psychotherapy, crisis intervention, care coordination, and case management.

Provides consultation and treatment support for innovative services, including Medication-Assisted Treatment (MAT) and chronic pain management, while delivering individual and group therapy. Serves as a resource to staff and providers by assisting with care coordination, problem-solving, and client support needs. Demonstrates a commitment to diversity, equity, and inclusion by fostering an inclusive and equitable environment and delivering culturally responsive services that enhance engagement with and understanding of the communities served.

• Provide direct client care, including behavioral health screenings and assessments, individual and group therapy, brief therapy, case management, care coordination, and related clinical services.
• Serve as a clinical resource for building site(s), including medical providers, nursing staff, pharmacy, case managers, and community collaterals.
• Provide/facilitate a wide variety of clinical services including individual, family and group activities to achieve treatment plan goals.
• Collaborate with staff in planning, developing, implementing, delivering, and evaluating clinical services, including coordination with Clinical Managers and Program Supervisors.
• Conduct behavioral health screenings and assessments within the medical clinic setting, including reviewing treatment options and providing referral information.
• Coordinate ongoing care and supportive services across Summit Stone Health Partners, Community Partner Location, community agencies, and other external partners to ensure clients receive needed services and support continuity of care.
• Provide crisis intervention services, including crisis assessment, de-escalation, and coordination of referrals to higher levels of care, as needed.
• Partner with medical providers to conduct behavioral health assessments for clients identified as having behavioral health needs.
• Complete clinical documentation in accordance with agency, state, and regulatory requirements.
• Support and foster team values by collaborating with peers and clinic staff to provide assistance and remove barriers to client care.
