Signify Health, part of CVS Health, seeks a Per Diem Clinician (NP or PA) to deliver in-home health evaluations, visiting members at their homes and helping bridge gaps in care. You will use an iPad and clinical workflow to review medications and medical history, perform a physical evaluation, and, if ordered by the plan, conduct additional diagnostic tests.
The role is part-time with a typical weekly 12 hours and travel within licensed states.
