In-Home Health - Nurse Practitioner or Physician Assistant Pueblo

Full‑time Clinician (Nurse Practitioner or Physician Assistant)

Signify Health, part of CVS Health®, is seeking a full‑time clinician to provide In‑Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care.

The visit lasts about 45 minutes and includes a medication and medical history review, a physical evaluation, and additional diagnostic tests requested by the member’s health plan (e.g., diabetic eye exams, spirometry). Clinicians use iPads and an intuitive clinical workflow. You are not required to prescribe medicine, order lab tests or alter current treatment regimens.

• Provide compassionate, patient‑centered evaluations in members’ homes.
• Conduct virtual visits as needed.
• Deliver all Signify Health services, including Diagnostic & Preventive Services products such as spirometry or diabetic eye exams.
• Build trusted relationships to guide members toward appropriate care and support their health goals.
• Travel locally and to licensed states within the coverage area as required.

Clinicians work in members’ private residences. Environments may include household pets, secondhand smoke or other allergens. Candidates must be able to perform duties safely in these settings.

• Pueblo / Fremont County (and surrounding licensed states)

• Active, unrestricted license(s) in the coverage area(s).
• Board certification (required).
• Multi‑state license(s) (preferred).
• Active NP or PA credentials.

• Pay range: $87,035.00 – $ (base salary varies by experience, education, and geography).
