Job SummarySocial workers are cross‑trained to provide care in multiple areas, including home visits, admission and imminent visits, and palliative care. The hospice social worker offers psychosocial support, skillful intervention, resource navigation, education, counseling, discharge planning, and assistance with end‑of‑life arrangements. The role requires city‑wide driving, home‑based care, and weekend availability.
Hospice Social Worker Responsibilities- Develop and maintain therapeutic relationships.
- Provide comprehensive psychosocial assessments.
- Develop and maintain comprehensive plans of care.
- Deliver supportive counseling.
- Facilitate smooth transitions from care environments.
- Mobilize community resources.
- Provide effective patient/family/caregiver education.
- Support patients and families through death and dying.
- Collaborate with interdisciplinary team to deliver patient care.
- Create timely and accurate documentation.
- Assure patient safety.
- Utilize resources efficiently.
- Adhere to Bloom standards and continuously improve processes and services.
- Maintain and enhance professional skills.
- Uphold high standards of personal and professional conduct.
- Allocate 50% of caseload to home‑based patients.
Minimum Qualifications
- Master's degree in social work from an accredited school and 1–3 years of palliative care or hospice experience.
- Demonstrated ability to assess patients' and families' emotional and social needs.
- Capacity to administer psychological/emotional assessments and develop treatment plans based on results.
- Excellent communication and interpersonal skills.
- Effective collaboration with interdisciplinary teams.
- Solution‑driven, creative, and resourceful problem‑solving abilities.
- Independent functioning with proactive anticipation of needs and problems.
- Skill in organizing and prioritizing work.
- Strong time‑management skills with attention to detail, accuracy, and follow‑through.
- Professional image, positive attitude, enthusiasm, and self‑motivation.
- Comfortable maintaining professional boundaries.
- Proficient computer skills.
- Broad understanding of community resources, including financial benefits.
- Awareness and sensitivity to cultural diversity.
- Knowledge of self‑care, personal boundaries, limits, and inner resources.
- Current Colorado driver’s license and automobile insurance.
