OCCUPATIONAL THERAPIST – FULL TIME – SOUTH DAVIS – BOUNTIFUL, UT
South Davis in Bountiful, UT is a post‑acute care facility offering long‑term care, short‑term care, and geriatric services. We provide a stable and supportive in‑house therapy team and excellent programs for our geriatric clientele.
We are currently seeking a Full Time Occupational Therapist for our in‑house rehab program.
Duties- Effectively screen and evaluate patients with cognitive and/or ADL physical functioning disorders and develop an appropriate plan of care following all regulatory and clinical practice standards.
- Utilize standardized assessments and evidence‑based practice to support clinical interventions.
- Provide rehabilitative, skilled and medically necessary treatment interventions to patients with cognitive and/or ADL physical functioning disorders per regulatory and clinical practice requirements.
- Administer various procedures as part of the rehabilitation plan, including ADL retraining, therapeutic activities, therapeutic exercises, modalities, use of supportive and assistive devices, etc.
- Assess patient progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
- Evaluate treatment outcomes for effectiveness; services are modified in relation to patient evaluation and referrals are made accordingly.
- Provide consultation and counseling to patients, families, caregivers and other service providers related to the physical disorders.
- Effectively communicate with supervisor and other health team members regarding patient progress, barriers, and treatment plans.
- Provide comprehensive discharge summary of services provided per regulatory and clinical requirements.
- Occupational Therapist license required.
- Open to Occupational Therapists at all experience levels; new graduates welcome.
