South Davis in Bountiful, UT is a post-acute care facility offering long‑term care, short‑term care, and geriatric services. The facility provides a stable, collaborative in‑house therapy team and excellent programs to support our geriatric clientele on their recovery journey.

• Effectively screen and evaluate patients with cognitive and/or ADL physical functioning disorders and develop a plan of care in accordance with regulatory and clinical practice standards.
• Utilize standardized assessments and evidence‑based practice to support clinical interventions.
• Provide rehabilitative, skilled, and medically necessary treatment interventions for patients with cognitive and/or ADL functioning disorders per regulatory and clinical practice requirements.
• Administer various procedures as part of the rehabilitation plan, including ADL retraining, therapeutic activities, therapeutic exercises, modalities, and use of supportive and assistive devices as appropriate.
• Assess patient progress and adjust treatments accordingly in the clinical record, meeting regulatory and clinical practice requirements.
• Evaluate treatment outcomes for effectiveness; modify services based on patient evaluation and refer when necessary.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding the physical disorders.
• Communicate effectively with supervisors and other health team members regarding patient progress, barriers, and treatment plans.
• Prepare comprehensive discharge summaries of services provided per regulatory and clinical requirements.

• Occupational Therapist license is required.
• Open to Occupational Therapists at all experience levels. New graduates are welcome!
