About This Role

As a Registered Nurse in Home Health, you will provide and direct nursing care to patients in their homes, coordinating with healthcare professionals to ensure continuity of care. This role is ideal for those who are passionate about delivering high‑quality patient care in a home setting.

• Pay: $64,100 - $141,500 annual total cash target pay
• Location: Bourne, MA

• Initiate, develop, implement, and revise the plan of care in collaboration with the physician and other healthcare professionals.
• Supervise care provided by home health aides and licensed practical/vocational nurses.
• Provide required supervisory visits.
• Deliver high‑quality clinical services within the scope of practice and infection control standards.
• Complete comprehensive assessments (OASIS) including medication reconciliation accurately and timely.
• Document patient visits per policy and payer requirements.
• Make initial and/or comprehensive nursing evaluation visits to ensure home health eligibility.
• Communicate relevant information timely and effectively with agency staff and other clinicians.
• Assist in the orientation of new agency personnel and serve as a preceptor.
• Participate in on‑call and weekend rotation as needed.

• Education: Current and unrestricted RN licensure in state of practice
• Experience: 1+ years of Home Health experience preferred
