Home Health Registered Nurse

About This Role

As a Registered Nurse in Home Health, you will provide and direct nursing care to patients in their homes, coordinating with healthcare professionals to ensure continuity of care.

• Pay: $41.71 - $62.57 hourly rate
• Location: Bourne, MA
• Facility: CAREtenders Bourne

• Schedule: Full Time

• Initiate, develop, implement, and revise the plan of care in collaboration with the physician and other healthcare professionals.
• Supervise care provided by home health aides and licensed practical/vocational nurses, providing instruction and assigning tasks.
• Provide required supervisory visits.
• Deliver high-quality clinical services within the scope of practice and infection control standards.
• Complete comprehensive assessments (OASIS) including medication reconciliation accurately and timely.
• Document patient visits per policy and payer requirements.
• Make initial and/or comprehensive nursing evaluation visits to ensure home health eligibility.
• Communicate relevant information timely and effectively with agency staff and other clinicians.
• Assist in the orientation of new agency personnel and serve as a preceptor.
• Participate in on-call and weekend rotation as needed to meet patient needs.

Education:

• Current and unrestricted RN licensure in state of practice

Experience:

• 1+ years of Home Health experience preferred

• Current Driver's License and vehicle insurance required
• Current CPR Certification or ability to complete within 90 days of hire preferred
