Job Description

Purpose of Position :
Responsible for day-to-day operations and clinical management of the Wound Centers. This position will oversee all functions of the clinical team. In addition, this position is responsible for developing and conducting ongoing education and process improvement for the clinic team with the assistance of the Director.

Description

1. Day to day management of the wound care team as it relates to workflow, staffing, and coordination of patient care according to patient acuity and volume.

2. Ensures staff provides quality safe care according to current standards of practice for Wound Care in the administration of procedures/treatments/medications.

Assists the Director in assuring Joint Commission standards are adhered to.

3. Keeps up to date with best practice standards in Wound Care and provides ongoing education to the team where needed.

4. Develop, mentor and coach continual improvement in the collaborative practice between physicians, nurses and the ancillary staff, along with the community partners.

5. Analysis and review of the program metrics using a working knowledge of the reporting capabilities in the EHR with a focus on wound healing metrics and coordination of care of the patients.

6. Provides efficient and effective care with the guidance of the physician during the clinic sessions, attends the clinics where scheduled and is dependable.

7. Rounds on a regular basis in each clinic to assure operational workflows are adhered to .

8. Point person for outside entities such as home care agencies and DME vendors to assure smooth transitions of care and educational opportunities where needed.

9. Ensures staff effectively provides patient/family teaching.

10. Ensures staff documents according to entity/dept. guidelines.

Is proficient with computer systems and assists with optimizing the HER as it relates to the clinic workflow.

Works with the IT team to implement any changes.

11. Addresses age-specific needs of school age, adolescents, adults, older adults.

12. Carries out Management responsibilities in accordance with the organization's policies and applicable laws.

13. Assists Program Director with interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems and keeping Program Director informed of all issues needing further review. .

14. Performs other related duties as assigned.

15. Challenges current working practices; identifies process improvement opportunities and presents recommendations and solutions to management. Engages and commits to the organization’s culture of continuous improvement by actively participating, supporting, and promoting CCHC Pillars of Excellence.

Qualifications

Bachelor's Degree preferred.
1-2 years of supervisory or management experience preferred.
Wound Certification preferred, certification required within 2 years of hire.
Five years’ experience in the treatment of chronic wounds required.
Current RN licensure in MA.
Current BLS certification.
