Who You Are You’re a licensed Nurse Practitioner with a passion for serving older adults and those with complex mental health needs. You thrive in an independent, flexible environment andare motivated by purpose, empathy, and progress.
Your Role As a Nurse Practitioner at PGS, you will deliver compassionate psychiatric care to residents in skilled nursing andlong-term care settings. You will work autonomously while staying connected to a robust clinical and administrative support team.
Key Responsibilities- Conduct on-site psychiatric assessments and medication management
- Coordinate care and consult with facility teams (Administrator,Director of Nursing, Social Services)
- Adhere tostate and federal clinical guidelines and PGSprotocols
- Educate staff and support holistic, patient-centered care
- Maintain clear, timely documentation

Requirements- State licensure as a Nurse Practitioner (MD, DC, VA, orPA required)
- Active DEA license
- Monday–Friday availability during normal business hours
- Ability to standfor 20 minutes and lift up to 15 pounds
