Position: Nurse Practitioner (Full Time) State Traveler - In-Home Health Assessments

Overview

$7,500 Sign‑On Bonus

Matrix’s national network of clinicians gauges members’ needs directly in their homes, helping older adults and other at‑risk individuals improve health outcomes and identify chronic conditions that might otherwise go undiagnosed.

Responsibilities

During a visit of up to one hour, providers review and observe a member’s current health, medical history, medication adherence, social environment and other risks, providing insights that are difficult to capture during routine office visits.

  • Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, physician referrals, case‑management referrals, follow‑up and clear documentation according to Matrix guidelines and protocols.
  • Administer and order point‑of‑care and lab‑based testing as needed.
  • Collaborate with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.
  • Work with the Primary Care Physician (PCP) on patient education and follow‑up as necessary.
  • Provide services at the member’s place of residence, which may be a home or a skilled nursing facility.
  • Qualifications

    Skills & Experience:

    • Master’s Degree or commensurate experience and satisfactory completion of NP licensure.
    • Current NP licensure in the state of practice, including prescription authority, or the ability to obtain prescriptive authority as needed.
    • Board certification from AANP, AACN, or ANCC in one of the following specialties:
      Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care.
    • Current BLS, ACLS, or CPR certification.
    • At least one year of NP experience.
    • Ability to travel up to 80‑90% of the time, including overnight travel and weekends within the assigned state and possibly neighboring states.
    • Valid state driver’s license for the state of residence.
    • Automobile insurance coverage required for the state of residence (or ability to obtain).
    • Strong computer skills and familiarity with employee health/medical record software.
    • Excellent verbal and written communication skills.

    Work Environment

    Full‑time providers typically work Monday through Friday from 8:00 a.m. to 5:30 p.m., with assessments offered Sunday‑Saturday from 8 a.m. to 7 p.m.
