About the Position
Specialty accepted:  Nurse Practitioner, Physician Assistant (Adult, Family, and Geriatric accepted)

Start date or shifts:  ASAP – ongoing

Daily schedule:  Monday–Sunday, 7:00 AM–7:00 PM | 5–8 in-home assessments per day

Practice setting:  In-Home Assessments

Types of procedures:  Health and wellness assessments, including H&P evaluations, medication reviews, vitals, lab screenings (finger stick, urine sample), and fecal kit distribution.

Number of patients:  5–8 per day

Ages:  Geriatrics

Additional information:  Providers must be available at least two full days per week (not necessarily consecutive) and include one weekday; weekend-only availability is not accepted. Work is not location specific, with opportunities available throughout most counties statewide. Assignments are scheduled based on provider availability and county needs, excluding only designated restricted counties. Travel and lodging benefits include hotel coverage for travel beyond 75 miles, rental car for providers completing 30+ weekly assessments, and flight reimbursement for assignments of at least 10 consecutive workdays.

Weekly $ incentive available for meeting visit goals. Ideal for independent providers who want to set their own schedule, choose where and when they work, and directly control their income through tiered pay per completed assessment. Be your own boss while providing meaningful, patient-focused care. Credentialing is self-paced, typically 30–45 days.
