In-Home Nurse Practitioner or Physician Assistant Part Time - Wheeling

Job Overview

Signify Health, part of CVS Health®, is seeking a Part‑Time Clinician (Nurse Practitioner or Physician Assistant) to provide In‑Home Health Evaluations. You will engage with individuals in their homes, helping to bridge gaps in care and build trusted relationships.

• Conduct in‑home evaluations lasting approximately 45 minutes, including medication and medical history review, physical assessment, and optional diagnostic tests such as diabetic eye exams and spirometry.
• Use an iPad and an intuitive clinical workflow for each evaluation.
• Provide services such as spirometry, DEE, bone density screening, and other Diagnostic & Preventive Services products as indicated.
• Conduct virtual visits as needed.
• Travel locally and within licensed states as required.
• Maintain professional conduct in diverse home environments, ensuring safety and comfort for both clinician and patient.

• Active, unrestricted license(s) in the coverage area(s).
• Board certification (required).
• Multi‑state licenses (preferred).

Part‑time employees qualify for select benefits, malpractice insurance coverage, and supplies. Anticipated weekly hours: 29.5.

Typical pay range: $41.85 - $90.13 hourly. Final offer will consider experience, education, geography, and other factors.
