Location: Warren County and surrounding service areas in Kentucky

Pay: $50/hourly
Coverage Area: Warren, Edmonson, Hart, and Butler Counties
Lifeline Home Health of Warren is seeking an experienced and compassionate Registered Nurse (RN) to join our growing home health team. This field-based position is ideal for an RN with prior home health experience who is comfortable managing an independent caseload and completing OASIS assessments. The successful candidate will provide skilled nursing care to patients in their homes while collaborating closely with interdisciplinary team members to deliver exceptional patient outcomes.
Schedule& Hours
- Weekly Hour Guarantee: 32 hours

Hours:
Monday through Friday, 8:00 AM to 4:30 PM - Start and end times may vary based on patient scheduling and field visits.
- On-Call Requirement: 2 to 3 nights per week
- Weekend Rotation:
One weekend every 3 to 4 weeks
- Perform comprehensive skilled nursing visits in patients' homes.
- Complete Start of Care (SOC), Resumption of Care (ROC), Recertification, Discharge, and other OASIS assessments accurately and timely.
- Conduct routine nursing visits and patient evaluations.
- Develop, implement, and update individualized plans of care.
- Monitor patient conditions and communicate changes to physicians and care team members.
- Provide patient and caregiver education regarding disease management, medications, treatments, and safety.
- Maintain accurate and timely documentation in accordance with agency, state, and federal guidelines.
- Coordinate care with therapists, physicians, office staff, and other clinicians to ensure quality patient outcomes.
- Active and unrestricted Registered Nurse (RN) license.
- Eligible to practice nursing in the state of Kentucky.
- Previous home health experience required.
- Strong knowledge and experience with OASIS documentation.
- Ability to independently manage patient visits with minimal orientation.
- Valid driver's license, reliable transportation, and proof of insurance.
