Elevance Health is seeking an LTSS Service Coordinator (Case Manager) to manage a designated caseload in Ohio’s LTSS programs. This field-based role requires travel to client sites with occasional office attendance, coordinating person-centered plans and collaborating with medical directors and interdisciplinary teams.
You will conduct assessments, document preferences and goals, develop PCSPs, and support families and caregivers while ensuring compliance with state and federal regulations.
