JOB TITLE:
STNA / Home Health Aide
- Minimum of 1-year paid experience in home careis required (experience with home health agency desired)
- STNA or CNA is preferred.
- HHA, PCA, DSP/DODD with 2+ years caregiving experience also considered.
LOCATION + TRAVEL:
Wakeman, OH and surrounding areas in Wood County
- Willingness to travel to/from client in Wakeman, OH is required for Part-Time employment (10-20hrs/wk)
- Willingness to travel to/from client in Wakeman, OH and throughout Huron County and/or up to 35 minutes from your home is required is required if additional hours are desired.
EMPLOYMENT TYPE:
Part-Time (14hrs/wk to start) + desire forLong-term commitment (2+ years at HCN)
- Must be willing and able to start immediately with 14 hours per week on Mondays and Wednesdays / daytime shift
- 30-day build-up to additional Part-Time or Full-Time (32-40 hrs/wk) hours is available (with add'l travel) if desired (not required)
SCHEDULE:
IMMEDIATE availability needed for PT: 14 hrs per week on Mondays and Wednesdays (exact start + end times are customizable).
- Additional case hours may be available for Part-Time or Full-Time employment on Weekday, Weekend, and hybrid schedules.
STARTING PAY RATE: $16 - $21/hr
Pay Rate based on certifications, experience, schedule and travel availability, work history, references, and overall fit.
Home Care Network, Inc. is URGENTLY SEEKING an experienced, compassionate, and professional Home Health Aide (STNA or CNA highly desired | experienced HHA, CHHA, DSP, or PCA will also be considered) to join our home health agency and provide hands-on personal care and support to a double-client (husband and wife) in Huron County, OH.
Responsibilities- Personal care, hygiene and Activities of Daily Living (ADLs) including safe transfers, and mobility/positioning support
- Support safe use of assistive devices (walkers, wheelchairs, lifts) - as needed
- Nutrition & Meal / Meal Prep Support
- Homemaking & Environmental Support (IADLs) including light housekeeping, errands, and tasks required to maintain a clean, safe, and organized home environment
- Community Engagement & Appointment Support, Coordination, and (in approved situations) Transportation
- Basic Health and Safety Monitoring
- Care Plan Implementation
- Dementia & Behavioral Support (if applicable)
- Demonstrated ability to follow a written Plan of Care
- Pro-active observation and excellent electronic documentation
- Supporting client independence, safety and confidentiality
- Professional, caring, and compassionate behavior to clients, client families, and your peers / co-workers

- Immediate start with 14+ hours per week (Part-Time):
- Mondays and Wednesdays daytime shifts are required.
- Additional case hours available for more Part-Time or Full-Time employment on Weekday, Weekend, and hybrid schedules.
- Local travel within Huron County | Expanded cases/hours may include adjacent Counties
- Minimum of 1 year paid in-home care or caregiving experience (agency experience desirable but not required)
- STNAs and CNAs highly desired (or HHA/PCA/DSP with 2+ years consistent experience)
- Exceptional on-time and attendance record, and consistent availability aligned to your chosen schedule type (PT/FT/Hybrid).
- Active CPR + FA certification and documentation of both
- Character traits: reliable, honest beyond reproach, professional, compassionate, detail-oriented (in care + documentation), accountable, friendly and accommodating, respectful
- Willingness and ability to perform required physical demands (see below)
- Consent + Ability to pass a State & Federal background check (including fingerprints)
- 18yo + active Driver's License / State
- Reliable transportation to all scheduled visits, within full reach of stated travel radius
Skills & Qualifications (not required for current client but may be for future clients)
- Experience with:
- G-tubes / feeding support
- Seizure protocols
- Hoyer Lift
- Behavioral health or crisis response
- High School diploma or GED
- Active STNA (State Tested Nurse Aide) or CNA certification, or previous home health aide certification
- Previous home health experience (1-3 years)
- Familiarity with Medicaid waiver services

- No Requirements for On-Call shifts, Weekends, extensive travel distances, or overtime (when not desired).
- Friendly travel expectations: Typical daily travel consists of Up to 30-minute client travel radius (from your home).
- Up to 45- or 60-minute client travel radius (from your home) is highly desired (not required).
