Essential Functions- Conduct comprehensive client intakes, psychosocial assessments, and diagnostic evaluations to identify behavioral, emotional, and psychological barriers and determine appropriate services.
- Develop and implement individualized, goal-oriented treatment plans using evidence-based approaches in collaboration with clients and interdisciplinary team members, including coordinating referrals as needed.
- Provide individual and group counseling services through in-person and telehealth modalities to address mental health, behavior change, and weight management-related factors.
- Monitor client progress by evaluating measurable outcomes, adjusting treatment plans accordingly, and maintaining accurate clinical documentation in compliance with HIPAA.
- Facilitate therapeutic groups and collaborate with interdisciplinary teams while engaging in ongoing professional development to maintain licensure, clinical competency, and quality of care.
- Prepare, prevent, protect and respond to support our culture of safety.
- Works hours per week in accordance with offer letter or per attributes of position at transfer request. May be expected to work longer hours as necessary.

Master of Social Work or another related field

Licensed Independent Social Worker (LISW) or Licensed Professional Clinical Counselor (LPCC)
