Horizon Health is seeking a PRN (as needed) Nurse Practitioner or Physician Assistant for the Eastern Region. Under the direct supervision of the Chief Medical Officer, and with the guidance and support of a mentor, the NP or PA will provide care to patients of all ages in an outpatient setting. New graduates are encouraged to apply. The organization seeks a highly professional individual with strong communication skills who is committed to coordinating all aspects of primary care for the patients served.

Successful candidates must be licensed or be able to successfully obtain licensure in the State of South Dakota. Certification from an accredited organization is required or must be obtained prior to employment. The Eastern Region includes Howard, De Smet, Bryant, and Lake Preston.
