Location: Boynton Beach, FL

Schedule: Full-Time | 8:00 AM – 4:30 PM

Salary: $75,000 – $85,000 per year (based on experience)

Isles of Boynton Nursing and Rehabilitation Center is seeking an experienced and compassionate Social Service Director to lead our Social Services department. This key leadership role is responsible for ensuring the psychosocial well‑being of our residents while maintaining regulatory compliance and supporting interdisciplinary care planning.

• Reports noncompliance with policies, procedures, regulations, or breaches in confidentiality to appropriate personnel.
• Reports retaliation or discrimination concerns to HR or the Compliance Officer.
• Reports allegations of abuse, neglect, exploitation, misappropriation of property, or mistreatment immediately and cooperates fully with investigations.
• Protects residents from abuse and ensures adherence to resident rights.
• Reports occupational exposures and work‑related injuries immediately.
• Follows established infection control policies and procedures.
• Participates in life safety drills, emergency preparedness training, and fulfills assigned emergency roles.
• Completes all required training and competency validations.
• Conducts resident and/or responsible party interviews to obtain comprehensive social histories.
• Assists residents in securing clothing, personal items, and personal care supplies as needed.
• Coordinates transportation for medical appointments and discharge needs.
• Facilitates communication of resident needs in their primary language or method of communication.
• Acts as a resource for staff regarding behavioral interventions.
• Participates in Resident and/or Family Council meetings as requested.

• Participates with the Administrator in budget development.
• Attends and contributes to Care Plan meetings.
• Serves as a member of:
  • QAPI Committee
  • Behavior Management Committee
  • Department Head/Stand‑Up Meetings
• Attends mandatory in‑services and completes all continuing education and licensing requirements.
• Contributes to ongoing efforts to maintain and improve quality of care.

• Bachelor’s degree in Social Work or a related human services field (sociology, gerontology, psychology, rehabilitation counseling, special education, etc.).
• Minimum of one (1) year of supervised social work experience in a healthcare setting working directly with individuals.
• Nursing home experience required.
• Strong knowledge of long‑term care regulations and discharge planning.
• Excellent communication, leadership, and organizational skills.
