Center Well Home Health is seeking a Home Health Physical Therapist to help patients regain strength, mobility and independence in the comfort of their homes. You will work under the Clinical Manager to deliver personalized care focused on functional improvement and patient-centered outcomes.
You will assess patients, implement rehabilitation plans aligned with physicians, supervise PTAs and HHAs, document progress, and communicate outcomes to physicians, the care team, patients and families.
