The position is a Full‑time weekend Registered Nurse (RN) role with flexible scheduling;
Saturday and Sunday are required.

• Perform skilled nursing visits and complete coordination of client care.
• Serve as point of contact for all disciplines involved in providing care to patients.
• Oversee the frequency of visits for the episode.
• Consult with the physician and office as needed, providing details about patient care.
• Perform accurate OASIS collection, ensuring the medication profile remains current.
• Ensure lab values are reported to the physician in a timely manner.
• Attend weekly case conferences and monthly case manager meetings.

• Registered Nurse (RN) licensed in the state of employment.
• Minimum of one year of clinical experience is preferred.
• Demonstrated knowledge and skill in current nursing practice.
• Valid state driver’s license and automobile liability insurance.
