Setting: Skilled Nursing Facility / Long-Term Care

We are seeking an experienced RN MDS Coordinator to join our skilled nursing team in the Boynton Beach area. This role is ideal for an organized, detail-oriented nurse who thrives in long-term care and is passionate about ensuring accurate resident assessments, quality outcomes, and regulatory compliance.

• Coordinate and complete MDS assessments in accordance with federal and state regulations.
• Ensure timely and accurate completion of the Resident Assessment Instrument (RAI) process.
• Collaborate with nursing, therapy, social services, and other departments to develop and update care plans.
• Monitor Medicare and reimbursement documentation requirements.
• Maintain compliance with CMS guidelines and facility policies.
• Participate in quality improvement initiatives and survey readiness activities.
• Educate staff on MDS processes and documentation standards.

• Active Florida RN license required.
• Previous MDS Coordinator experience in a Skilled Nursing Facility preferred.
• Strong knowledge of RAI, Medicare, and CMS regulations.
• Experience with care planning and interdisciplinary team collaboration.
• Excellent communication, organization, and documentation skills.
• RAC-CT certification is a plus.
